How can my GP or other health professionals send you records?
We are happy to receive records via Medical Objects or by fax to 07 4927 4693. Your GP or other referrer can also upload the referral on Upload a referral tab.
How can I send you my information?
Any relevant information is helpful in assisting our clinicians come to the right diagnosis and treatment. Please see the Contact us tab for contact details of our clinic.
How can I request you to share information with myself or other health professionals?
We are happy to share medical information with the doctor/clinician chosen by the patient. For release of information requests to patients, the requests will be considered individually for every patient by their treating psychiatrists and then action taken based on their best interests. There is a $50 to $ 100 administration fee based on the quantity of information and administration time involved.
