Important Information for Your Initial Appointment if your GP referral is for ADHD

What to bring to your appointment? 

  • The referral from your GP or specialist. 
  • Any relevant reports or correspondence from previous treating psychiatrists, psychologists or GPs  
  • Any history of previous medications/treatments trialled 
  • A current medication list 
  • Any allergies. 
  • Medicare Card 

If you have an established diagnosis of ADHD, please bring your old medical records confirming the diagnosis. 

If not, the process of diagnosing ADHD may require up to 2 to 3 consults before treatment is initiated. 

Prior to your initial consultation you are expected to complete the following: 

Please have a copy of your BP, Pulse, BMI, Height, Weight and ECG done in last 6 months 

Blood tests as per your GP’s advice (usually patients require FBC, U and E, eGFR, LFT, TFT, fasting lipid profile and glucose, Vit B12, Folate, Vit D, Iron Studies) will also be required 

Please bring your school reports. It is important you also bring any psychologist, psychiatrist and paediatrician records. Please bring along or pass on the contact information for a family member or someone who knows you very well as a child and as an adult for the psychiatrist to obtain collateral information on the day. 

If there is an appraisal from work or if your supervisor has given any feedback about your work, kindly include that too.

Please note that non-availability of any of the above information is likely to cause delays in your assessment and treatment.

What to bring to your first appointment:

  • The referral from your GP.
  • Any relevant reports or letters.
  • Any history of previous medications/treatments trialed.
  • If the referral we received from your GP indicates a probable ADHD assessment, then we will need your primary and high school reports, a copy of your most recent ECG, copies of any previous assessments by psychologist/ pediatrician or other psychiatrist, any assessments completed for other neuro-developmental disorders like ASD. Online assessment tools will be emailed to you a week before your appointment. The email will look similar to the sample one below:
